Employee Tax Information

Notice of Availability of Form 1095-C 

In compliance with the Affordable Care Act (ACA), JLS Automation is required to make available to full-time employees the Form 1095-C, which provides information about your offer of health insurance coverage for the previous year.  

Form 1095-C is now available upon request.  In years past, we have mailed this document to employees directly.  The Form 1095-C is informational only and typically not needed to complete annual income tax filing obligations.

If you would like to receive a copy of your Form 1095-C, please contact the HR Department using one of the following methods:

Requests may be made at any time, and the Form will be provided to you by January 31 or within 30 days of your request, whichever is later.
